United code on catalog for show, can't get specials without it, Help. Thanks

Anyone get a United Catalog, I need the code #, on the catalog in order to get the show special.
This catalog or broshure was handed out at the show. I can't find mine.

Not sure where the "code" is. The only thing I see on our catolog is "cat. no. 508". Is that what you need?
