employment application form

I am in need of an employment application form, I've lost the only unfilled copy that I had. Does anyone out there have one that I could copy or use or direct me to a place where I can get a generic form?

You should be able to get a pad of generic application forms from Staples or some similar office supply store. If you want some forms that are specific to your store, then you could go through any number of printing businesses that do labels, letter head, etc.

If you have Word, you can find templates online which you can edit to fit your store.

I've always just used a piece of scrap mat board. I hand that and a pen to them and tell them to tell me about themselves. This gives me an aptitude and creativity test all in one.
