Model Income Statement

Hello all,
I've taken Goltz's class on Pricing and he hands out a model income statement to use as a guide. I can't find it now that I want to use it. Doesn anyone have a copy or know where I can get one (even if it isn't the one he created)?
I'm trying to figure out a budget. I'm currently thinking of getting out on my own, instead of working for the man!
